Venous Ulcers

New Technique to Heal Venous Ulcers: Terminal Interruption of the Reflux Source (TIRS).
 

The treatment of venous ulcers continues to be a challenge. The compression therapy aid to prevent recurrence but fail to heal ulcers. This technique named TIRS targets the terminal source of reflux. Venous ulcers develop because of the venous hypertension at the level of the ulcer. The TIRS procedure targets the venous hypertension, thus allowing healing to occur.

How do venous ulcers develop?

Venous ulcers are the  result of skin breakdown due to failure of preventing the consequences of venous diseases. When the pressure is greater than 45mmHg in the veins it increases the risk of forming ulcers.

 

On several occasions patients provide a history of trauma as the inciting event, but poor wound healing ensues owning to underlying venous hypertension due to underlying  reflux or obstruction or both.

How do we diagnose venous ulcers?

The typical location is in the so called ‘GAITER “ area around the inside of the ankle. It can also develop on the outside also.  Venous ulcers respond very quickly to appropriate compression therapy. Failure to respond to compression stockings should create suspicion it could be a possible skin cancer.

How do we treat Venous let Ulcers

Treatment of these ulcers begin with appropriate local ulcer care , elemination of serious infection(Debridement in theatre) and proper moisture balance.

 

Historically the single and standard therapy was elastic compression stockings but healing rates were traditionally

40-50% at 6 months  and the recurrence rates may be as high as 50%. 

 

Recently minimally invasive EVLA( Endovenous Laser Ablation)  have been used adjunct in treatment of venous ulcers and have shown to improve the rate of healing in patients.

 

Using the T.I.R.S (Terminal Interruption of the reflux source) we have been able to achieve rapid healing in patients who had this procedure. This causes an abrupt reduction of venous pressure enabling ulcer healing.

 

Rapid healing of all venous ulcers with an average time to ulcer healing of 3 months.
